Referrals & Admissions
Admission to CEI
If you are interested in CEI, please contact our Director of Services, Carol Barth, at 770-962-3908. You can schedule a tour of the facilities and learn about the process of applying for admission through the available referral sources.
NOW and COMP Waiver Programs
Individuals who hold waivers under the New Options Waiver (NOW) and Comprehensive Supports Waiver (COMP) programs may apply for services at Creative Enterprises. Georgia's Medicaid waiver program is administered by the Georgia Department of Behavioral Health and Developmental Disabilities (DBHDD). Individuals who are Medicaid eligible may apply for a waiver through DBHDD. Please click here for more information from DBHDD on how to access services.
Available Medicaid Waiver services include:
- Community Access Group (T2025-HQ)
- Community Access Individual
(T2025-UB)
- Prevocational Services (T2015)
- Supported Employment Individual (T2019-UB)
- Supported Employment Job Maintenance (T2019-HQ)
The type and amount of services you receive from Creative Enterprises will depend on your individual waiver budget.
DOL Vocational Rehabilitation Program (VR)
We are accepting referrals for individuals through the Georgia Department of Labor Vocational Rehabilition Program (VR). Please click here to find the nearest Rehabilitation Services office.
If you are a VR counselor, click here to download our Referral Form. The form is in .PDF file format, which can be opened using the latest version of Adobe Reader or Foxit Reader (both programs are free to download).The form allows you to input information directly by typing into the fields and selecting the appropriate check boxes. Please note that if you are using the free version of Adobe Reader you may not be able to save any information that you input to the form, so you must print the completed form or risk losing information. Foxit Reader will allow you to save your inputs, so you have the option of sending your referral form via email.
